Health and Wellness Survey Available

Dear Colleagues:

In spring 2012 the University’s Board of Trustees (BOT) asked me to direct the Office of Institutional Research, Demography & Assessment to conduct a health and wellness survey of faculty and staff, which includes opinions regarding smoking behavior and related rules, laws or policies.

The results of this survey will be reported to the BOT, internal administrators and other internal interested parties, including students. The results of the survey may be used to examine current health-related policies and support services at EWU. I hope that you will take just a few minutes to respond to this important survey.

Please know your participation in this survey is completely voluntary and that your responses are anonymous as they do not require you to disclose any identifying information. You are not required to answer any particular questions in the survey and can exit at any time by closing your browser.
