Intent to Graduate Form

All graduate students are required to complete the Intent to Graduate form below to begin the formal final review of your file.

Deadlines

Form must be submitted by the initial deadline for your name to appear in the Commencement Bulletin. If you miss these deadlines, your name will not appear in the Commencement Bulletin.

There will be a $15.00 late fee for all Intent to Graduate Forms that are submitted during the same term you plan to graduate.

Quarter Student Deadlines:

  • February 1 – Spring and Summer Quarter
  • June 1 – Fall Quarter
  • November 1 – Winter Quarter

Semester Student Deadlines:

  • February 1 – Summer Semester
  • June 1 – Fall Semester
  • November 1 – Spring Semester

Graduation Evaluation and Checklist

This request begins your final evaluation for graduation. This evaluation includes:

  • Review of candidacy requirements
  • Review of grade point average
  • Verification of enrollment of 2 credits
  • No remaining ‘x’ or ‘y’ grades (excluding all 600/601/602)
  • Paid Graduation Fee
  • Verification of Graduate Committee & Option Change Form (only pertains to students doing a thesis or research project)

You will receive a checklist of your evaluation results sent to the email provided below.

Final oral defense examination scheduling will not be permitted until an Intent to Graduate has been submitted.
